I certify the statements in this document are accurate and consent to the release of any information contained herein to the Ministry of Transportation, CAASCO, Insurance Bureau of Canada and the MTO Course Inspector. A non-refundable deposit is required to hold your seat, the balance is due on the first day of class. A $15 BDE Certification Fee (no tax) is charged by the Ontario government. The fee is included in your payment. Class change request made less than 7 days prior to first day of scheduled class subject to availability and $35 administrative charge.
